The Hospice Registered Nurse Case Manager assesses the physical, psychosocial, and environmental needs of patients and their families. They coordinate care through an Interdisciplinary Plan of Care and provide support and education on symptom management and bereavement.
Candidates must be a graduate of an accredited nursing program with a current State RN license and at least one year of clinical care experience. A valid driver's license, reliable transportation, and the ability to lift up to 50 pounds are required.
