Requires a PharmD and an ASHP-accredited PGY-1 residency or three years of clinical experience, with current Delaware licensure. A PGY-2 in Ambulatory Care and BPS board certification are preferred.

This position may work remotely up to two (2) days per week.
As the Population Health Clinical Pharmacist, you will provide Population Health Management Services to patients of the Medical Group of ChristianaCare. The Clinical Pharmacist is responsible for completing comprehensive medication reviews for eligible patients, as well as ensuring that performance targets are met or exceeded for medication-related measures across value-based contracts. Collaborates with providers and patients to ensure patients are meeting applicable measures.

Hourly Pay Range: $58.26 - $93.22This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting.
