This position involves collaborating closely with physicians, nurses, and other healthcare professionals to assess, diagnose, and manage acute and chronic illnesses. The APP will be responsible for developing and implementing individualized treatment plans, performing procedures within scope of practice, and ensuring continuity of care through effective patient education and follow-up. This role demands a strong commitment to evidence-based practice, patient safety, and continuous professional development to improve clinical outcomes. Ultimately, the APP contributes significantly to the efficiency and effectiveness of the healthcare team, enhancing patient satisfaction and overall health system performance.
