Administer physical therapy treatment plans and therapeutic modalities to patients within their homes. Document client records and coordinate with the Physical Therapist and Branch Director to ensure quality care.
Requires a current state license to practice Physical Therapy, a valid driver's license with auto insurance, and CPR certification. Candidates must be fluent in English and possess knowledge of geriatric population changes.
As a Home Health Physical Therapist Assistant, you will:
Required Qualifications:
Current state license to practice Physical Therapy
Valid driver's license, auto insurance and reliable transportation
Current CPR certification
Experience collaborating with a team of healthcare professionals
Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population
Read, write, and speak fluent English
Preferred Qualifications:
One year of experience as a Physical Therapy Assistant
Pay Range
$41.00 - $57.00 pay per visit/unit

TB Statement:
This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driving Statement:
This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
