The Registered Nurse Case Manager plans and directs personalized hospice care, conducting comprehensive assessments and developing care plans for patients and families. They coordinate interdisciplinary services and provide professional nursing care to ensure dignity and comfort during the end-of-life journey.
Candidates must be a graduate of an accredited nursing school with current state licensure and CPR certification. A bachelor's degree and experience in acute, home, or hospice care are preferred.

We are centered on caring for the whole person – with expert medical care, pain management as well as emotional and spiritual support along with resources, information, and emotional support for families and caregivers.
The Registered Nurse Case Manager plans, organizes, and directs hospice care and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
