The Licensed Practical Nurse will collaborate with RNs and physicians to provide direct patient care, including medication reconciliation and specialized procedures. They are responsible for teaching self-care techniques to patients and caregivers while maintaining accurate and timely clinical documentation.
Candidates must hold a current LPN license for the assigned state and possess at least one year of clinical experience. A valid driver's license, reliable transportation, and current CPR certification are also required.
Verify plan of care with the RN prior to each visit.
