The Medical Behavioral Social Worker assists in developing medical and behavioral health care plans while providing support and guidance to Medical Management staff. They facilitate communication between multidisciplinary teams, members, and providers to ensure effective discharge planning and community resource utilization.
Candidates must hold a Bachelor's Degree in Social Work and possess a current LBSW or higher licensure in the State of Texas. A minimum of 2 years of experience in medical social work is required.
Under general direction of the Manager of Behavioral Health and Social Work, the Medical Behavioral Social Worker assists with development of medical and behavioral health plans of care and services to patients, families and the community, for the social, emotional, psychological, economic, and environmental issues related to the entire continuum of care. Provides support and guidance to Medical Management staff members related to Behavioral Health and Social Services issues. This position is expected to maintain strong working relationships with members, providers and other health plan staff.

In early 2022, Central Health adopted a Healthcare Equity Plan which will guide the immediate and long-term work of the hospital district to improve health and reduce inequities for Travis County’s low-income population. The plan identifies four strategic imperatives: Access & Capacity, Care Coordination, Member Engagement, and System of Care.
