Description

Position Overview

The PrEP Navigator provides patient-centered navigation, education, outreach, and care coordination services for individuals who may benefit from HIV prevention services, Pre-Exposure Prophylaxis (PrEP), HIV/STI testing, sexual health education, and related supportive services. This role helps reduce barriers to prevention of care by supporting appointment scheduling, health insurance navigation, medication access, adherence support, follow-up, referrals, documentation, and community outreach. The PrEP Navigator reports to Sr. Director of Clinical Services, and collaborates with outreach & education, case management, and community partner teams to promote equitable access to culturally responsive HIV prevention services.

Mission

To improve health and wellbeing, Allies provide integrated medical care, supportive human services, and community-based education for individuals living with, or at risk of HIV, viral hepatitis, and sexually transmitted infections.

Vision

Allies envision a healthier community whereby people access integrated medical care and supportive human services in a respectful setting, free of stigma and discrimination.

Key Responsibilities

· Provide individualized care coordination for patients starting, continuing, or re-engaging in PrEP services at Allies, including support with medication adherence.

· Help patients schedule and attend appointments by providing reminders, following up on missed visits, and arranging transportation support.

· Assist patients with insurance enrollment, medical assistance, copay assistance, and other coverage applications, including patient assistance program enrollment.

· Coordinate referrals to internal and external services, including primary care, behavioral health, STI services, and case management.

· Facilitate group PrEP education sessions for priority populations, including HIV/STI testing support and PrEP enrollment during sessions.

· Provide accurate, affirming education on PrEP, PEP, HIV/STI prevention, harm reduction, safer sex practices, and routine screening.

· Develop relationships with community partners, referral sources, and organizations serving communities impacted by HIV.

· Maintain timely, accurate, and confidential documentation of patient contacts, referrals, outreach, follow-up, and services provided.

· Enter and track required data in the electronic health record, prevention database, grant reporting system, and other designated tools.

· Follow agency policies related to confidentiality, HIPAA, documentation, mandatory reporting, safety, and professional boundaries.

· Participate in meetings, case conferences, quality improvement activities, training, and program planning as assigned.

Requirements

Knowledge Skills and Abilities

The PrEP Navigator should have proven experience in the following areas:

· Experience providing patient navigation, outreach, case management, health education, community health services, HIV/STI prevention services, or related direct service support preferred. 

· Knowledge of HIV prevention, PrEP, PEP, STI testing, harm reduction, sexual health, and community-based public health services preferred. 

· Ability to communicate respectfully and effectively with diverse communities, including LGBTQ+ individuals, communities of color, people living with or affected by HIV, people who use substances, and individuals experiencing barriers to care. 

· Strong organizational skills, diligence, documentation skills, and ability to manage follow-up tasks on time. 

· Ability to maintain confidentiality and handle sensitive information with professionalism and discretion. 

· Proficiency with Microsoft Office, electronic records, email, calendars, and data tracking systems. 

· Ability to work some evenings or weekends as needed. 

Preferred Qualifications:

· Certification or training in PrEP Navigation, HIV testing, STI counseling, community health work, motivational interviewing, trauma-informed care, harm reduction, or related practice areas. 

· Experience working in an HIV service organization, public health department, community-based organization, or healthcare setting. 

· Familiarity with insurance navigation, medication assistance programs, benefits screening, and referral coordination.

High school diploma or equivalent required; associate degree, bachelor’s degree, or relevant certification in public health, human services, social work, health education, community health, or related field preferred. 

The PrEP Navigator may be required to perform physical activities associated with general office work, including standing or sitting for extended periods. Current PA child abuse, PA criminal history, and federal criminal history clearances, along with verification of a TB test completed within the past year, are required within the first 30 days of employment.

Compensation for this position is $21.63/hour-$25/hour. We comply with EEOC.
