The Case Manager conducts client assessments and develops individual service plans to help clients achieve successful outcomes. They provide supportive counseling, link clients to essential services, and document all activities according to agency guidelines.
Candidates need an Associate's degree in Human Services or a High School Diploma/GED with two years of relevant shelter experience. Proficiency in basic computer skills and professional communication is required, with Spanish language skills preferred.

The Case Manager is a key provider of services within Volunteers of America’s Case Management Model. The Case Manager works with the client to assess their strengths and needs, plan a course of action, link the client to services, counsel the client and monitor progress. Case Manager must assure that clients receive appropriate, competent and professional information and advice, referrals and advocacy services to achieve successful outcomes.
