This is an incredibly exciting time to join our service. We have launched Living Better Lives in Lancashire our new strengths-based vision for Adult Social Care. We are looking for a Social Worker to support us to deliver our vision of supporting people to achieve what matters to them, living independently in a place they call home.

This post is a weekend working position, Friday to Monday (29.6hours).
The role
Joining our Short Term Support Service as a Social Worker in this role, you will support safe, timely and person-centred discharges from hospital, working closely with health colleagues, families and wider partners. You will help people move from hospital to the most appropriate setting, promoting recovery, independence and a positive discharge experience.
