Job Title: Family Care Advocate
Location: Northeast Scottsdale Area
Hours & Schedule: Monday-Friday, 8:00 AM-5:00 PM. Onsite presence is required at least once per week in the Northeast Scottsdale area.
Work Environment: Hybrid work environment supporting adolescents ages 12-17 and their families. This position requires a combination of remote work and at least one onsite day per week.
Travel Required: Local travel may be required based on family and program needs.
Salary / Hourly Rate: $54,000-$62,000 annually
Benefits Offered: Health, dental, vision, PTO, cell phone, tuition reimbursement for education in a related field.
On Call Required: Yes. This position participates in a 1:4 rotating on-call schedule. On-call responsibilities do not require onsite presence.

Job Summary:
The Family Care Advocate provides direct support, advocacy, education, and resource coordination for adolescents ages 12-17 and their families. This role serves as a consistent point of support for families, helping them navigate services, identify needs, overcome barriers, and connect with appropriate resources.
The Family Care Advocate works collaboratively with adolescents, family members, and members of the care team to promote engagement and continuity of care. The position requires strong interpersonal and communication skills, sound judgment, appropriate professional boundaries, and the ability to respond effectively to changing family needs.
Job Duties & Responsibilities:
Build supportive, professional relationships with adolescents ages 12-17 and their families.
Serve as an advocate and resource for adolescents and family members throughout their care journey.
Assist families in identifying needs, barriers, goals, and appropriate community or care-related resources.
Provide education, guidance, and practical support to help families navigate available services.
Maintain regular communication with adolescents, families, and appropriate members of the care team.
Coordinate resources and support services based on individual and family needs.
Encourage family engagement and participation in recommended services and care plans.
Help families address barriers that may interfere with accessing or continuing services.
Maintain timely, accurate, and professional documentation of interactions and activities.
Protect confidentiality and maintain appropriate professional boundaries.
Collaborate effectively with team members and other professionals involved in supporting adolescents and their families.
Participate in team meetings, case discussions, and other collaborative activities as required.
Respond professionally and appropriately to urgent family needs within the scope of the position.
Participate in a 1:4 rotating on-call schedule with no onsite requirement while on call.
Maintain at least one onsite workday per week in the Northeast Scottsdale area.
Perform additional duties consistent with the responsibilities of the Family Care Advocate role.
Previous experience working with adolescents ages 12-17 and their families is required.
Strong interpersonal, communication, advocacy, and relationship-building skills.
Ability to communicate effectively and compassionately with adolescents and family members.
Ability to maintain confidentiality and appropriate professional boundaries.
Strong organizational and time-management skills.
Ability to work collaboratively as part of a multidisciplinary team.
Ability to participate in a 1:4 rotating on-call schedule.
Ability to work onsite at least once per week in the Northeast Scottsdale area.
