The Case Manager is responsible for utilization reviews, resource management, and developing safe, timely discharge plans for critically ill patients. They also facilitate multi-disciplinary team meetings and negotiate reimbursement to maximize insurance benefit coverage.
Candidates must hold a current clinical license as a Nurse, Respiratory Therapist, or Social Worker. Previous experience in an inpatient hospital setting dealing with critical or acute care patients is required.
Requires a current licensure in a clinical discipline either as a Nurse (RN/LPN/LVN) or a Respiratory Therapist (RT) OR Medical Social Work (license per state guidelines).

Previous discharge planning experience preferred.

You will report directly to the Director of Case Management and provide social work services, as necessary, per state guidelines.
