The Clinical Care Coordinator provides holistic, wrap-around support by conducting bio-psychosocial assessments and developing individual care plans for mental health participants. They also coordinate access to evidence-based interventions and maintain partnerships with clinical and community stakeholders to ensure integrated service delivery.
Candidates must be qualified mental health professionals such as Social Workers, Nurses, or Occupational Therapists with AHPRA registration or relevant professional membership. Demonstrated experience in complex mental health settings and the ability to travel across the Toowoomba region for outreach work are required.
Join a passionate, multidisciplinary team delivering person-centred mental health care as part of the new Toowoomba Integrated Mental Health Hub, an innovative partnership between Each, Wellways, Queensland Program of Assistance to Survivors of Torture and Trauma (QPASTT) and Goondir Health Services.
Your role, your impact
As a Clinical Care Coordinator, you’ll provide holistic, wrap-around support for people living with or at risk of mental illness. You’ll conduct bio-psychosocial assessments, develop individual care plans, and coordinate access to evidence-based interventions such as CBT, DBT, ACT and Motivational Interviewing.
This role is pivotal in ensuring seamless, integrated care that addresses both mental and physical health needs, empowering participants and their families with choice, control and connection to ongoing supports.
