About the role
Deliver vital nursing care, clinical assessment, and education to patients across the hospital’s Diabetes Centre, Coordinated Care Clinics, High Risk Foot Clinics, and inpatient wards.
Provide clinical nursing care to diverse patients with type 1, type 2, and complex diabetes conditions
Partner closely with endocrinologists, dietitians, and diabetes educators in a high-volume setting
Conduct clinical assessments and assist with complex insulin management plans
Help patients prevent long-term complications and manage their health effectively
Coordinate care pathways, acute triage, & community referrals for a culturally & socially varied patient population
About you
Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency
Demonstrate a minimum of 2 years post graduate experience and demonstrated clinical experience in acute diabetes care and chronic disease management
Experienced in working within a multidisciplinary healthcare team in high-volume clinical setting
Working knowledge of continuous glucose monitors (CGM) and insulin delivery systems
Strong interpersonal skills to support patients from diverse socio-economic backgrounds
