DESCRIPTION:
A Nutrition Care Manager conducts comprehensive in-home assessments with elders, determines need and eligibility for home delivered meals, and creates care plans.
Duties include data entry into computer database, communication and consultation with family members, other Springwell staff and community professionals.
The compensation for this role is $28.21/hr for a 37.5 hour work-week.

In July of 2022, we merged with BayPath Elder Services to increase our service area to more than 22 communities in Greater Boston and MetroWest. Our team of 200 case managers, information and referral specialists, protective service workers, social workers, and RN's are professional, dependable, and compassionate resources for the people we serve.
