The RN provides care and services to hospice patients in their homes or various healthcare facilities. They collaborate with an interdisciplinary team to perform assessments and interventions while managing a specific patient caseload.
Candidates must hold a current Registered Nurse (RN) license in good standing for the state of practice. A valid driver's license, reliable transportation, and the ability to use electronic medical records are also required.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, on-call or Resource role.
