Hinds Lifecare, one of the most well-known and well-respected nonprofit agencies in California’s Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a LVN Intake Coordinator to join our team.
The LVN Intake Coordinator is responsible for taking all hospice information calls regarding the intake of new referrals to facilitate an evaluation for hospice service. The LVN manages all patient intake functions throughout the entire intake process and provides Hospice evaluations as directed.

Qualifications
Compensation $35 - $38 hourly

Minimum 1 year experience in acute care facility or community health agency, (prefer one-year hospice experience).
Current LVN license, CPR, recent physical and TB, Tdap, MMR, Varicella, HepB immunizations.
Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs).
Ability to learn and assimilate the NetSmart Computer Program/System and navigate through multiple technical interface portals to enter and retrieve information related to patient care.
Ability to work weekend rotation.

OUR PROGRAMS Hinds Hospice operates the only freestanding six-bed hospice home in Fresno, Madera, and Merced Counties. When patients' symptoms become too complicated to be cared for in their home, the Hinds Hospice home in Fresno offers around-the-clock nursing care. The Hinds Hospice Center for Grief and Healing is unique in that it serves not only hospice families, but also the community at large with grief support services. We offer one-on-one grief counseling, group support specific to the type of loss, monthly grief mailings, or someone to listen on the phone. The news of an unborn baby being diagnosed with a life-limiting condition is heartbreaking. Angel Babies offers a perinatal hospice program designed for families who have received this diagnosis, offering support and comfort during pregnancy and providing ideas for creating loving memories. Children require different care than adults, so our innovative pediatric hospice program allows for concurrent treatment along with hospice services.
