The Discharge Coordinator manages aftercare coordination and treatment planning for behavioral health patients to ensure a smooth transition post-discharge. What we're looking for
Applicants should have a bachelor’s degree in Social Work, Counseling or equivalent required. Additional requirements include:
Prior experience with psychiatric and chemical dependency patients
Strong organizational, communication, and documentation skills required
