Candidates must have a high school diploma or equivalent and the ability to work with elderly individuals who may be physically or mentally ill. Strong organizational skills and knowledge of long-term care regulations are required.

Under the direction of the Social Services Director, our Social Services Designee will assist each resident and family in adjusting to placement, illness, and the plan of care, helping them attain the highest practicable level of functioning in accordance with current federal and state regulations, and our policies and procedures. You will assist residents and families with wishes regarding treatment and help with their transition and adjustment to the facility. You will also participate in resident and family council meetings as requested, and support Admissions and assist with discharge planning services.
