The Case Manager coordinates patient care from admission to discharge, overseeing interdisciplinary plan-of-care decisions and resource management. They facilitate team conferences, monitor patient experience, and ensure all required documentation and regulatory standards are met.
Candidates must be qualified to perform independent assessments (e.g., RN, SW, OT, PT, ST) and hold an active state license if required. A minimum of an Associate Degree in Nursing or a Bachelor's degree for other disciplines is required, with CCM or ACM certification required within two years.
