The Community Health Worker Service Coordinator performs outreach, assessments, and service coordination for Star Kids members to improve health outcomes. They are responsible for monitoring a specific group of members and providing ongoing education and support.
Candidates must have a high school diploma or equivalent and a Texas Community Health Worker certification. A minimum of five years of experience in community health or social services is required, along with bilingual proficiency in English and Spanish.
The CHW Service Coordinator is responsible for performing a wide range of activities for Star Kids members to positively affect individual health outcomes. The CHW Service Coordinator will monitor a group of level 3 members who do not meet the requirements for an LMSW or RN named service coordinator. Activities include, but are not limited to outreach, assessment, Member education, service coordination and Member support.
High school diploma or equivalent.
Minimum of five (5) years’ experience in community health, social services, community advocacy, community outreach, health plan Member Services and/or Engagement, or a combination of relevant experience and education.
Understanding of the community in the Service Delivery Area through shared experiences and/or a strong desire to help people in vulnerable communities.
Private, reliable transportation.
Bi-lingual (English/Spanish) Required. Expert proficiency in conversational, reading, writing, and translating medical terms as well as diagnoses. Must pass Spanish test prior to hire.
Excellent communication skills
Knowledge of basic medical terminology
Demonstrated ability to meet deadlines and manage competing priorities
Willingness to work in a frequently changing environment
Ability to establish and maintain positive and effective work relationships with coworkers, members, and providers.
Computer skills and experience with Microsoft Office products.
TX Community Health Worker certification.
