Candidates must hold a Bachelor's degree and possess at least three years of experience in case management, discharge planning, or social work. A Basic Life Support certification from the American Heart Association is required.

As our Medical Social Worker, you will coordinate and implement post-discharge plans in close coordination with Case Managers, utilizing the Electronic Medical Record (EMR) and Allscripts while assisting with advocacy and referrals to community resources. Every day you will manage patient discharge processes, provide essential advocacy for patients, and facilitate seamless referrals to critical community resources, ensuring continuity of care and comprehensive support. To be successful in this role, you will possess strong social work skills, expertise in discharge planning and community resource navigation, and the ability to effectively utilize EMR and Allscripts.
