Provide client-centered HIV case management services, including bio-psychosocial assessments and the implementation of multidisciplinary care plans. Coordinate access to community resources and provide supportive counseling to clients and families coping with illness.
Requires a bachelor's degree in a health-related field or equivalent case management experience. Basic Life Support (BLS) certification must be obtained within one month of hire.
The Case Worker I will work collaboratively with the multidisciplinary healthcare team to provide case management services to clients as assigned, including the following expectations:

Basic Life Support certification must be obtained within one month of hire date.
Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
Equivalent of a bachelor’s degree in a health-related field, such as social work, counseling, sociology, or psychology. Applicants with bachelor’s degrees in other fields may be considered with sufficient case management experience.
