The Care Manager will conduct monthly patient outreach to monitor chronic conditions, ensure medication adherence, and coordinate care plans. They will also document encounters in the EMR and collaborate with providers to support patient health goals.
Candidates must hold an active Medical Assistant certification and have at least three years of clinical experience with chronic conditions. Proficiency in EMR systems and strong communication skills are required for this remote role.

Be a part of a dynamic remote Team of MA's lead by an RN under the supervision of physicians to conduct monthly calls to support your patients with complex chronic conditions, helping them manage their health and stay connected to their care team between regular office visits to achieve their maximum health potential.
