At Mercy Housing, we're redefining affordable housing by creating stable communities where everyone has the opportunity to thrive. As the nation's largest nonprofit affordable housing provider, we build, preserve, and manage service-enriched housing across the country.
The Community Health Promoter will play a lead role in educating community members on health promotion and disease prevention activities. Develop and coordinate projects that directly respond to community needs and priorities. Conduct community outreach and work closely with residents, community shareholders and Community Health Promoter Lead (CHP) to identify and address health barriers and disparities.
This is an on-site position. We encourage individuals with lived experience to apply.
Pay: $20-22/hour, dependent on experience. This is a part-time role, approximately 11 hours per week.
Duties & Responsibilities
Assist with the implementation and integration of the health and wellness initiatives outreach project to champion effective healthcare for vulnerable and disadvantaged low-income individuals.
Coordinate and conduct health education and promotion activities with community groups of all ages; target outreach to groups and individuals who are experiencing language and cultural barriers to healthcare services.
Enter and record data accurately and timely into SIMS and other identified data recording tools. Report outcomes of regularly scheduled on-site programs and service.
Develop and maintain a comprehensive list of available community resources and partners. Establish and maintain working partnerships with relevant organizations and individuals, with an emphasis on providing on-site services to residents and multiple communities.
Works with other Resident Services and Housing Property Staff and Partnering Entities to conduct health education activities, promote and coordinate programs, share best practices, support cross-group collaboration and identify underserved individuals or groups.
Prioritize, organize, and accomplish work assignments. Take on responsibilities, be reliable and dependable, report to work on time and complete all scheduled hours. Facilitate and lead effective community meetings and activities.
Recruit, motivate, and coach community volunteers and leaders to support the activities and advocacy of the health and wellness initiatives grant.
Collect information and document incidents that could potentially need follow-up by property staff and/or residents involved.
Competencies
Educate and assist individuals and groups on how to use available community health care resources.
Focus on connecting health services with individual and community needs.
Facilitate health care, social service systems, key service providers and healthcare delivery systems become culturally relevant and responsive to the health and welfare needs of residents and surrounding communities.
Minimum Qualifications
Two (2) years of experience in health care.
Preferred Qualifications
Bachelor’s Degree in Healthcare Management, Public Health, Social Work, or Community Organizing.
Bilingual; Spanish, Somali, Amharic, Ukrainian, Russian, and/or Asian dialect).
Experience translating, interpreting, and documenting for individuals, groups, and health care/social service providers.
Knowledge and Skills
Communicate effectively both verbally and written to a diverse group of residents, staff, external contacts and across all lines of authority while maintaining compliance with privacy laws.
Works well with others in a team environment, respecting and valuing the perspectives and contribution of others.
Intermediate proficiency in Microsoft Office Suite software.
