The nurse will conduct face-to-face assessments of newly admitted members in acute care settings to facilitate safe transitions from hospital to home. They will collaborate with healthcare providers to identify care gaps, educate families on discharge planning, and ensure accurate clinical documentation.
Candidates must hold an unrestricted Florida Registered Nurse (RN) license and possess at least 3 years of recent acute care nursing experience. Proficiency in discharge planning, care coordination, and basic computer applications is required, along with the ability to commute to the specified facility.
Onsite Location
This is an Onsite / Hybrid position at the (Ascension Sacred Heart Pensacola - 5151 N 9th Ave) located in (Pensacola, Florida). The ideal candidate must live within a reasonable commute to the facility preferably no more than a 30-60 minute driving distance to the Onsite location from their home base.
This position will work at an Onsite facility and as a Hybrid; will have limited working hours at home based on the leader’s discretion.
