The Nurse Care Manager provides care coordination, including in-home assessments and the development of patient-centered service plans. They monitor member needs across the healthcare continuum to ensure quality, cost-effective outcomes and safe discharge planning.
Requires an Associate's degree in Nursing (Bachelor's preferred) and at least two years of experience as a Registered Nurse. Must hold a current New York RN license and a valid driver's license with insurance.

In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
