The Case Manager will partner with participants to develop individualized recovery plans and provide community-based services to support their goals. They will also serve as an advocate, maintain documentation, and participate in a multidisciplinary team to ensure holistic care.
Candidates must hold a Bachelor's degree in Human Services or a related field and possess at least two years of relevant experience. A valid driver's license and the ability to pass criminal background checks are also required.
