Coordinates day services and develops Individual Support Plans (ISP) for individuals with intellectual disabilities. Oversees health and safety, manages electronic health records in CareLogic, and serves as a liaison between families and the plan team.
Requires a degree (Associate's, Bachelor's, or Master's) with corresponding experience (4, 2, or 1 year respectively) working with persons with intellectual disabilities. Must possess a valid driver's license and be able to pass criminal record and child abuse clearances.
Overview:
This an exciting position that coordinates day services for individuals with intellectual disabilities, including leading the development of the Individual Support Plan (ISP), ensuring that supports are provided as per the ISP, and completing required documentation.
This position works with staff to develop and implement activities that support the individual in achieving their desired outcomes. This role requires creativity, self-initiation, record management proficiency, solid problem solving skills, and the ability to work well with all members of the Plan Team.
