Work hours: PRN (As Needed), schedule can vary depending on the unit; Monday-Friday, 8:30am-5:00pm or 10:00am-10:30pm and Saturday & Sunday, 8:30am-5:00pm. This position requires 2 weekend days/month minimum and 1 critical holiday staffing.

Our central location provides convenient care to people in the DFW Metroplex and across North Texas and Oklahoma. With more than 1,000 employees and 450+ physicians on our medical staff we are one of Denton County’s largest employers.
Texas Health Denton is a Joint Commission-accredited Primary Stroke Center and Chest Pain Center and a Bariatric Surgery Center of Excellence and a Pathways to Excellence-designated hospital. We are the only choice in Denton for oncology services, women and infants’ services, and much more.
Here’s What You Need
· Master's Degree Social Work is required upon hire. (Individuals hired as a CTSW prior to May 11, 2017, will be grandfathered to the CTSW position with BSW, at the entity they were employed at on May 11, 2017)
· Three (3) years of hospital/medical social work experience is preferred.
· One (1) year of discharge planning/care management experience is preferred.
Licenses and Certifications
· LMSW - Licensed Master Social Worker or LCSW - Licensed Clinical Social Worker is required upon hire.
· CPR - Cardiopulmonary Resuscitation is required upon hire.
· ACM - Accredited Case Manager is preferred upon hire.
· CCM - Certified Case Manager is preferred upon hire.
· Other ANCC is preferred.
In addition to the qualifications, a successful Care Transition Manager - Social Worker - will model the factors listed below:
Working knowledge of medical necessity criteria preferred
Knowledge of Microsoft Outlook and Office (Word, Excel)

Ability to engage in complex clinical decision-making
