The Navigator provides case management and direct outreach support to high-risk individuals with mental health or co-occurring disorders. Key duties include developing support plans and acting as a liaison between members, families, and community agencies to coordinate treatment.
Candidates must have a High School Diploma or GED and a valid driver's license with reliable transportation. A minimum of 2 years of experience or lived experience working with homelessness, justice involvement, or serious mental illness is required.
Open Sky’s Community Supports Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting.
The Community Services Navigator will provide case management and direct outreach support to their assigned caseloads, which consist of members enrolled in Community Services Programs, Recovery Support Navigation, Behavioral Health Justice Involved and/or Community Support Program for People Experiencing Chronic Homelessness.
