The Licensed Practical Nurse will provide high-quality nursing care to patients in their homes according to the established Plan of Care. They are responsible for documenting patient progress, communicating changes in condition to the clinical team, and assisting patients with self-care techniques.
Candidates must hold a current State License as a Licensed Vocational or Practical Nurse and have graduated from an accredited nursing program. At least one year of clinical experience is required, with preference given to those with hospice or home health backgrounds.
