The Licensed Vocational Nurse will conduct home health visits to monitor patient vital signs, perform medication reconciliations, and execute specialized procedures. They will collaborate with RNs and physicians to ensure patient care plans are followed and documented accurately.
Candidates must possess a current LVN license and at least one year of clinical experience. A valid driver's license, reliable transportation, and current CPR certification are also required.
