Job Overview We are seeking a compassionate and reliable Complex Care Support Worker (Driver) to join Yorkshire Community Care. The role involves providing high-quality, person-centred support to adults with complex needs, including safe transportation to appointments and community activities. The successful candidate will work as part of a multidisciplinary team to promote independence, dignity and positive outcomes for the people we support.
Key Responsibilities
Provide safe, respectful and person-centred support in line with individual care plans, promoting independence, choice and dignity for service users.
Safely drive service vehicles to transport service users to appointments, activities and community visits, adhering to all road safety laws and organisational driving policies.
Assist with mobilisation, manual handling and safe transfers using appropriate techniques and equipment, ensuring the safety and comfort of service users at all times.
Support personal care tasks where required, following care plans and maintaining privacy and dignity.
Administer and support medication in accordance with organisation policies, training and care plans, and maintain accurate medication records where applicable.
Contribute to assessment, care planning and review processes by providing accurate and timely observations, reporting changes in needs and participating in multidisciplinary meetings as required.
Maintain accurate records and complete daily logs, handovers and incident reports in line with organisational procedures and confidentiality requirements.
Promote positive relationships with service users, families, carers and external professionals, communicating clearly and respectfully at all times.
Adhere to safeguarding policies, reporting any concerns promptly and cooperating with investigations as required.
Ensure vehicles are kept clean, secure and in good working order; report any maintenance issues and follow vehicle inspection checklists.
Attend and participate in mandatory training, supervision and team meetings to maintain professional competence and support continuous improvement.
Support a culture of equality, diversity and inclusion, ensuring care is culturally sensitive and tailored to individual needs.
Person Specification
Previous experience supporting adults with complex needs or in a similar care role is desirable, demonstrating an understanding of person-centred practice.
Valid UK driving licence with appropriate entitlement for the vehicle used; a clean driving record is preferred. Willingness to undertake role-related driving checks and provide driving history.
Knowledge of safe manual handling techniques and willingness to undertake Moving & Handling training and use of equipment as required.
Ability to administer or support medication in line with training and organisational policy is desirable.
Previous experience in managing tracheostomy and ventilation (Cpap)
Good communication and interpersonal skills, able to engage positively with people who use services, families and colleagues.
Reliable, flexible and able to manage changing priorities, travel between service locations and work occasional early starts, evenings or weekends to meet service needs.
Organised with attention to detail for record-keeping, reporting and maintaining vehicle check documentation.
Commitment to safeguarding, confidentiality and upholding the rights, dignity and independence of service users.
Willingness to undertake an enhanced DBS check and provide satisfactory references as part of pre-employment checks.
Commitment to continuous professional development and adherence to organisational policies and regulatory standards.
