Candidates should be proficient in medical terminology and EHR navigation with a preference for CMA, RMA, or LPN credentials. One or more years of related experience and bilingual skills in English and Spanish are desired.

Under the general direction of the Medical Director, the Clinical Coordinator supports the wellbeing of all Family Health Center patients by ensuring timely communication, action, and follow up on a wide variety of patient related concerns. The Clinical Coordinator functions as a liaison between patients and members of the care team through communication (via phone, fax, electronic health record, in-person, or otherwise), carrying out provider instructions (i.e., communicating lab results, refilling prescriptions, etc.), and maintaining meticulous documentation.
