The Case Manager collaborates with multidisciplinary teams to develop and monitor patient care plans and facilitate the discharge process. They are responsible for utilization reviews, coordinating with payors, and ensuring adherence to TJC and CMS accreditation standards.
Candidates must have a relevant bachelor's degree, current state clinical licensure, or 4 years of relevant experience, preferably in an acute care setting. A valid driver's license and a commitment to safety and infection prevention standards are also required.
The Case Manager works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process. This position must integrate company values into daily practice.
