The Community Engagement Specialist will lead the Connect for Life program by hosting events, conducting home visits, and facilitating member connections to improve health outcomes. They are responsible for documenting interactions, identifying community leaders, and ensuring members are supported in accessing healthcare services.
Candidates should have 3+ years of experience in community outreach, teaching, or facilitation, with a preference for a Pennsylvania Community Health Worker Certificate. An associate's degree is preferred, and applicants must be comfortable with remote work, possess reliable transportation, and demonstrate strong communication skills.
