Job Description Summary

Provides specialized support for children and families with complex needs by managing a caseload of children enrolled in Medicaid Waivers within the Case Management Agency (CMA). Oversees high acuity cases, facilitates transitions from children to adult waivers, and coordinates emergency and exception processes. Strengthens partnerships with families, community providers, and interdisciplinary professionals to support stability, promote service quality, and ensure cohesive care planning. Contributes to system improvements through specialized children's services expertise, training, and knowledge sharing.

Primary office location will be in the Greeley office. Work may involve time away from a standard office environment, which may consist of driving to and from home visits. Work may occur in various community settings that are potentially hazardous and may require additional precautions,
such as personal protective equipment and/or requesting assistance from emergency medical personnel or law enforcement. Reliable, predictable attendance within department business hours of 8 a.m. to 5 p.m. Monday through Friday. After hours and on-call work situations may occur, on occasion, to provide support to members and families that need services outside of normal business hours. This position has the opportunity for a hybrid schedule, for employees who have completed initial training and meet criteria outlined in division policy.

Job Description

Complex Case Management - 40%

  • Maintain a caseload of high acuity and transitioning children as assigned, including Children's Habilitation Residential Program (CHRP) and out-of-state placement cases.
  • Convene individualized support team meetings as needed to address case specific issues.
  • Provide mediation between members, families, and providers to resolve conflicts and support long-term and person-centered support planning.
  • Develop appropriate residential transitions for children/youth in out-of-home placements.
  • Coordinate RFP process for placement settings.
  • Support members and families enrolled in a children’s waiver during out of county and/or out of state placements.
  • Cultivate and sustain effective partnerships with key internal and external stakeholders (e.g. Child Welfare, Family Resource Division, Regional Accountable Entities (RAE) -Creative Solutions Team and provider-based wrap facilitators) to support integrated service planning and high-quality outcomes.
  • Collaborate closely with Weld County School Districts (based on the child's resident school location) to ensure smooth, well-coordinated educational transitions.
  • Work with members, families, and community agencies to coordinate and facilitate services.
  • Conduct out-of-county travel, when necessary, to support coordinated case management for CMA members/children that are also involved with other DHS programs.


Waiver Transition Coordination - 15%

  • Coordinate all children’s transitions to adult waivers, including assessments, planning meetings, documentation, and implementation of transition plans.
  • Facilitate and manage Developmental Disabilities (DD) Medicaid Waiver enrollment requests.
  • Facilitate and oversee all emergency requests and exception processes for children transitioning to the DD waiver.
  • Provide information as requested about all potential services and the enrollment process, guiding, and prompting through each step as needed.
  • Ensure accurate, comprehensive, and consistent documentation to the state.


Advocacy, Family and Provider Support - 20%

  • Participate in Family Engagement, Team Decision Making, and Creative Solutions meetings.
  • Strengthen and maintain effective collaboration with community partners, providers, and interagency partners.
  • Serve as subject matter expert for high-acuity needs and child-to-adult transitions.
  • Advocate for the best interest of the member.
  • Mediate with members, families and providers during conflict situations and assists with problem solving to identify the best solutions to meet members’ long term supportive needs including potential, future needs that may arise.
  • Ensure the members and families are notified of rights, that discussion of priorities occurs, and that they are given choice of services based on documented needs and goals.
  • Remain current on all applicable rules and regulations that may impact children’s case management and represent the agency at required local and state meetings, committees, workshops, and conferences.


Training & System Support - 15%

  • Conduct specialized children’s services training as requested.
  • Support system improvements through training contributions and knowledge sharing.
  • Model high quality, person-centered service delivery, demonstrating expected interactions, practices and standards.
  • Support colleagues by troubleshooting complex cases and provide clear guidance on navigating program requirements, promoting consistent and effective case management practices.


Professionalism & Teamwork - 10%

  • Participate in a positive and value-added manner with both internal and external professional partners.

Required Qualifications

Required Education

  • Bachelor's Degree or 5 years experience in the field of Long-Term Services and Supports (LTSS), which includes Developmental Disabilities or
  • A combination of education and relevant experience appropriate to the requirements of the position. Relevant experience is defined as: (a) Experience in one of the following areas: long-term care services and supports, gerontology, physical rehabilitation, disability services, children with special health care needs, behavioral science, special education, public health or non-profit administration, or health/medical services, including working directly with persons with physical, intellectual, or developmental disabilities, mental illness, or other vulnerable populations as appropriate to the position being filled; AND (b) Completed coursework and/or experience related to the type of administrative duties performed by case managers may qualify for up to two (2) years of required relevant experience.


Preferred Experience

  • Experience managing complex Medicaid waivers and/or Long-Term Care cases and developing and maintaining effective, collaborative relationships with families and professional partners.
  • Strong skills in conflict mediation, meeting facilitation, and collaborative problem-solving.
  • Experience in children’s case management, child protection, or related high acuity service delivery.
  • Familiarity with transition processes for children entering Medicaid Adult Development Disabilities Waivers.


Skills and Abilities

  • Candidate must possess knowledge of Long-Term Services and Supports Case Management practices, developmental disabilities services, and children’s waiver programs (CES, CHRP, CLLI, CWCHN, CFC).

Licenses and Certifications

  • Driving is essential in this position and will be traveling throughout the state.
