The Case Manager provides outreach and support to individuals and families experiencing homelessness to help them access medical, mental health, and housing services. They are responsible for developing case plans, coordinating with community partners, and documenting progress in HMIS and EHR systems.
Candidates must have a Master's degree in Social Work (or related field) with under 5 years experience, a Bachelor's with over 5 years experience, or 9 years of field experience without a degree. A valid driver's license, personal vehicle, and liability insurance are required for travel within the service area.
