Pride in North Carolina, LLC is seeking a Facilitator for our High Fidelity Wraparound program to engage high-need youth and families in developing and monitoring individualized care plans.
Position Summary
The Facilitator ensures the principles and phases of the High-Fidelity Wraparound process are delivered with fidelity to the model, supporting families in achieving positive outcomes.
Key Responsibilities
Engage youth and families in care planning and monitoring. Develop integrated plans and natural support systems. Maintain strong relationships with families and community partners. Ensure fidelity to High-Fidelity Wraparound principles. Document all activities accurately and timely.
Education & Experience
• Required: Bachelor’s degree in human services or related field (experience may substitute).
• Two years of experience providing services for children, youth, and families.
