The Social Service Director provides medically related social services to ensure the physical, mental, and psychosocial well-being of residents. Key duties include developing care plans, coordinating community resources, and managing discharge planning in collaboration with an interdisciplinary team.
Requires a Bachelor's Degree in Social Work or Human Services with at least 2 years of supervised experience in a healthcare setting. A Master of Social Work (MSW) and experience with PCC software are preferred.

This job has no supervisory responsibilities.
Education and/or Experience
Bachelor’s Degree in Social Work or in Human Services and 2 years of supervised social work experience in a health care setting working directly with individuals. MSW preferred
Language Skills
Ability to read technical procedures.
Ability to read and comprehend policy and procedure manuals.
Ability to effectively present information and respond to questions from managers and employees.
Mathematical Skills
Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
Reasoning Ability
Ability to solve practical problems.
Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Certificates, Licenses, Registrations
Knowledge and experience with PCC preferred.
