New Horizons is a nonprofit organization dedicated to serving individuals with special needs. With over 70 years of experience, we are committed to enhancing their quality of life and promoting their independence and inclusion in the community.

The Community Living Services (CLS) Coordinator supports daily program operations through administrative, operational, and field-based assistance. This role ensures high-quality, compliant, and person-centered services for Members while supporting Life Skills Instructors and families.

Partnering with the Program Manager, the Coordinator manages scheduling, staff training, documentation, billing support, and case coordination. Additionally, they maintain an assigned Member caseload and provide direct support when necessary to ensure continuity of care.

Position Type: Full-Time, non-exempt

Schedule: Monday – Friday (8:00 AM to 4:30 PM). Must have availability to work evenings and weekends as Member needs arise.

Pay Rate: $25.00

Program Operations & Coordination

  • Supports the implementation of the Community Living Services program by promoting the agency's mission, values, and person-centered service philosophy.
  • Assists the Program Manager with daily program operations, staffing coordination, and scheduling to ensure authorized service hours are consistently met.
  • Monitors Member authorizations, service utilization, attendance, billing documentation, and timekeeping to promote accurate service delivery and reimbursement.
  • Assists with staff coverage by coordinating schedule adjustments and providing direct support services when necessary to maintain continuity of care.

Member Services & Case Coordination

  • Maintains an assigned caseload and serves as a resource to Members, families, and support by facilitating communication and coordinating services.
  • Collaborates with Members, families, Regional Center staff, and interdisciplinary team members to develop, implement, and monitor Individual Service Plans (ISPs) that reflect each Member's goals, preferences, strengths, and support needs.
  • Assists in developing meaningful community-based activities and instructional supports that promote independence, self-determination, community inclusion, and achievement of ISP outcomes.
  • Monitors Member progress and ensures assessments, quarterly reports, progress notes, incident reports, and other required documentation are completed accurately and within required timelines.

Documentation & Quality Assurance

  • Conducts routine field visits to observe service delivery and ensure supports are provided in accordance with each Member's Individual Service Plan (ISP), Regional Center requirements, and agency policies.
  • Monitors staff performance in the field and provides immediate coaching, constructive feedback, and recognition to promote continuous professional development.
  • Documents field observations, coaching provided, identified concerns, corrective actions, and follow-up activities, ensuring timely communication with the Program Manager.
  • Ensures staff are implementing ISP objectives consistently, collecting accurate data, and utilizing approved teaching strategies and interventions.
  • Maintains complete, accurate, and confidential Member records in accordance with agency policies and regulatory requirements.
  • Oversees the organization, maintenance, and electronic filing of Member records to ensure documentation is current, readily accessible, and audit-ready.
  • Reviews progress notes, attendance records, EVV documentation, billing records, and staff documentation for accuracy, completeness, and compliance.
  • Assists in maintaining employee documentation, including performance evaluations, corrective action records, acknowledgments, and required personnel forms.
  • Ensures agency databases and electronic documentation systems are utilized consistently and accurately by all staff.

Staff Support & Training

  • Assists with onboarding and training new employees and helps ensure staff complete all required agency, program, and regulatory training.
  • Provides ongoing support to newly hired staff during their onboarding period through field mentoring, competency observation, and coaching to ensure successful integration into their role.
  • Provides technical assistance, coaching, and ongoing support to Direct Support Professionals and Life Skills Instructors regarding documentation, technology, service delivery, and agency procedures.
  • Coordinates with the Information Technology department to resolve technology-related issues affecting staff and program operations.
  • Assists in monitoring staff compliance with required certifications, training completion, and documentation standards.

Compliance & Continuous Quality Improvement

  • Supports compliance with Regional Center requirements, agency policies, and applicable state and federal regulations governing Community Living Services.
  • Coordinates Member, family, and staff satisfaction surveys and assists leadership with analyzing survey results to identify opportunities for quality improvement.
  • Participates in internal quality assurance activities, audits, corrective action efforts, and continuous improvement initiatives.
  • Identifies operational challenges and recommends process improvements that enhance efficiency, compliance, and quality of services.

OTHER DUTIES AND RESPONSIBILITIES including the following:

  • Attends meetings and trainings as directed by CLS Program Manager.
  • Supports the Agency’s mission and partners with others to accomplish the mission.
  • Maintains the highest professional and ethical standards at all times.
  • Supports other Community Living Services team members as needed to ensure the highest level of quality services and customer service.
  • Maintains current certification in first aid and CPR.
