LSSI is hiring a full-time Care Coordinator, Monday through Friday from 9:00am to 5:00pm for the Pathways to Success Program. The Care Coordinator will be responsible for providing high quality care coordination for LSSI’s Pathways to Success Program clients and to ensure increased engagement and access to the care they want and need using a person-centered, family-centered, recovery-oriented approach.
