Job Summary
The Case Manager position advocates for the patient and ensures the patients are receiving all needed services and resources, as outlined in the treatment plan or plan of service, and ensures those services are efficiently coordinated.
Minimum Requirements:
Education: Bachelor’s degree in a behavioral science or related degree
Licensure/Certification: Valid Driver’s License and auto insurance with good driving record. Able to obtain a class E driver’s license and meet KVC’s requirement for auto insurance
Experience: Experience in social services, residential treatment, education, or another related field. Previous experience working with children/families in a trauma informed environment
Preferred Experience/Skills: Requires intermediate skills in Microsoft Office suite, including Word, Excel and Outlook email. Strong customer service focus, excellent time management, organizational, oral and written communication skill. Must be
proficient in Word, Excel, and email applications
Key Responsibilities
Ensures resources/services are available for the patients as outlined in the patients’ plan and documents those resources/services
Facilitates communication and reporting amongst all pertinent team members, both internal and external
Engages in crisis intervention with youth
Arranges or provides transportation youth to community appointments
Meets with youth at least weekly or as specified
Maintains and documents weekly contact with agencies/parents
Coordinates and communicates with other departments for scheduling and treatment
Maintains an updated agency mailing list
Prepares information for psychiatric medication reviews
Schedules staffing reviews for all youth on the caseload as specified. Clearly communicates the schedules to all treatment team members, both internal and external
Participates in team meeting, department meetings, etc.
Coordinates admissions, discharges, and transfer of caseload
Meets with parents, caseworkers, and guardians to complete paperwork as well as conducts assessments, gathers patient documentation, distributes patient information to the appropriate staff, and enters client data into electronic health record. Ensures patient is supported through transition into the program.
For discharges, ensures all paperwork and notifications are in order
Other duties as needed for the betterment of Camber.
Additional duties may be assigned based on organizational needs and staff qualifications.
Physical & Work Environment
The conditions described are representative of those required to perform this role, with reasonable accommodations available. This position involves regular standing, walking, and communication, with frequent use of hands and occasional bending, kneeling, or climbing. The role may require lifting up to 10 pounds regularly, 25 pounds frequently, and 75 pounds occasionally. Standard visual abilities are required. Work is performed in a moderate noise environment with occasional exposure to outdoor conditions.
