About the Role
As a Recovery Navigator at the Northumberland Recovery Partnership, you will play a vital role in supporting and guiding individuals throughout their recovery journey from substance misuse. Working as part of a multi-disciplinary team, you will provide collaborative recovery planning and case management to individuals accessing support for drug and alcohol use, from initial assessment and engagement through to sustained recovery.
You will build strong, supportive relationships with service users, offering encouragement, motivation and person-centred support to help individuals achieve positive outcomes and increase their recovery and social capital within the community.
What You'll Do
Manage a caseload of service users, developing, implementing and reviewing personalised recovery and treatment plans.
Conduct triage, screening and comprehensive assessments to identify individual needs and risks.
Provide one-to-one support sessions and deliver evidence-based psychosocial interventions, including motivational interviewing and CBT-informed approaches.
Facilitate access to treatment, support services and community resources, including counselling, education, vocational training and peer support opportunities.
Lead effective case management, including risk assessments, safeguarding plans and risk management processes, ensuring accurate use of online case management systems.
Work collaboratively with partner agencies, families and multi-disciplinary teams to support progression from engagement through to self-sustained recovery.
Develop and deliver group work and tailored recovery programmes that meet local needs.
Support harm reduction initiatives, including drug screening, BBV testing, immunisation provision and community needle exchange services where appropriate.
Monitor and review service user progress, adapting recovery plans to meet changing needs.
Maintain accurate and up-to-date records, reports and documentation in line with organisational policies and procedures.
Promote visible recovery within the service and encourage positive family and peer involvement throughout recovery journeys.
To Succeed in This Role, You’ll Need
NVQ Level 3 in Health & Social Care (or equivalent) or willingness to work towards this qualification.
Relevant professional qualification in addiction studies, counselling, social work or a therapeutic discipline (desirable).
Experience or understanding of recovery planning, risk management and safeguarding practices.
Knowledge of and ability to deliver interventions such as Brief Interventions, Motivational Interviewing (MI), Psychosocial Interventions (PSIs), CBT and ITEP.
Understanding of harm reduction approaches relating to substance misuse.
Strong communication and relationship-building skills with the ability to engage and motivate individuals.
Ability to work collaboratively with internal teams, external professionals and partner agencies.
Proficiency in Microsoft Office programmes and experience using case management systems.
Ability to work flexibly across operational sites, including occasional evening and weekend working where required.
