Coordinate community-based palliative care services by managing patient registration, scheduling home visits, and linking patients with multidisciplinary teams. Facilitate referrals to government schemes and maintain detailed patient records and MIS data.
Requires a degree in Social Work, Sociology, Psychology, or Public Health with 1-2 years of experience in community health or NGO fieldwork. Candidates must possess strong empathy, communication skills, and the ability to travel locally within community settings.

Role Purpose
To support
the effective delivery of community based palliative care services by
coordinating community outreach, patient registration, follow-up visits, and
linkages between patients, caregivers, and the multidisciplinary palliative
care team.

Key Responsibilities
· Act as the primary community
link between patients, caregivers, and the palliative care team.
· Identify volunteers.
· Identify and register eligible
palliative care patients in the community in coordination with ASHAs, ANMs,
CHOs, and health facilities.
