JOB TITLE: Successful Young Parents Home Visitor
REPORTS TO: In-home Supports Supervisor

JOB PURPOSE
The Successful Young Parents Home Visitor plays a key role in delivering individualized, relationship-based support to young families (23 years old and younger) with young children. This position focuses on strengthening parenting capacity, promoting healthy child development, and enhancing family well-being through in-home and community-based services.
The Successful Young Parents Home Visitor works collaboratively with families, internal teams, and community partners to provide strengths-based, culturally responsive, and trauma-informed support. This role combines direct service delivery with program coordination, documentation, and community connection.
This role includes a shared onboarding and planning process with the Supervisor to establish expectations, caseload structure, and service delivery approach. This plan will be reviewed regularly to ensure alignment, responsiveness to family needs, and continuous improvement in practice.

HOURS
35 hours per week, typically Monday to Friday, 8:30 am to 4:00 pm (in addition to a 30-minute unpaid lunch break), with the option of a flexible work week. This position requires some flexing of hours and evening or week-end work to accommodate program and client needs.

LOCATION
This position is community-based, with work conducted in family homes, community settings, and Families Matter office locations.

QUALIFICATIONS:
Educational and professional requirements
•A degree or diploma in Social Work, Human Services, Early Childhood Development, or a related field is required
•Minimum of 2–3 years of experience supporting families with young children, including work in the areas outlined below

Knowledge, Experience and skills requirements
•Experience supporting families with complex needs
•Strong understanding of risk and protective factors in family systems
•Knowledge of early childhood development and attachment-based parenting approaches
•Experience delivering home visitation or community-based family support and group facilitation is an asset
•Strong oral and written communication skills
•Ability to work independently and as part of a collaborative team
•Strong organizational, documentation, and time management skills
•Proficient computer and data entry skills
•Knowledge of community resources and service systems supporting children and families
•Ability to engage families using a strengths-based, trauma-informed, domestic-violence informed and culturally responsive approach

Other Requirements
•Valid driver’s license and regular access to a reliable vehicle
•Updated Police Information Check and Child Intervention Check

DUTIES AND RESPONSIBILITIES:
Program Delivery (Home Visitation & Group Facilitation)

•Provide regular in-home and community-based visits to families using a strengths-based, domestic-violence informed, trauma-informed, and culturally responsive approach
•Build trusting relationships with caregivers to support parenting confidence and capacity
•Model positive parent-child interactions and promote healthy attachment
•Provide education on child development, parenting strategies, and family well-being
•Support families in setting and working toward individualized goals
•Respond to emerging family needs with flexibility and professionalism

Planning & Coordination
•Conduct assessments to identify family strengths, needs, and goals
•Develop and implement individualized service plans in collaboration with families
•Coordinate visit schedules and manage caseload effectively
•Prepare materials and resources tailored to each family’s needs
•Responsible for the planning, facilitation, and administrative duties of group programming. (Ex: Parent-child drop ins, client outings, parenting groups)

Program Monitoring, Documentation & Reporting
•Maintain accurate and timely case notes, documentation, and client records
•Track family progress and outcomes using program tools and frameworks
•Collect and input data to support program evaluation and reporting requirements
•Maintain records of weekly activities, visits, and client interactions
•Monitor progress on family goals and program deliverables
•Participate in team meetings, case reviews, reflective supervision, and regular check-ins with Supervisor
•Submit mileage claims, expense reports, and HR documentation in a timely manner
•Submit program-based success stories and other funder required documents

Community Engagement & Resource Navigation
•Connect families to community resources and services to reduce barriers and increase support
•Build relationships with community partners and service providers
•Support families in accessing programs such as health, mental wellness, housing, and early childhood services
•Participate in outreach and community engagement activities as appropriate
•Support with documentation to allow families to access benefits and other services.

Team Collaboration

•Work collaboratively with internal teams to ensure coordinated service delivery
•Maintain effective communication with supervisors and colleagues
•Contribute to a positive team environment and organizational culture
•Participate in cross-program initiatives and knowledge sharing

Training & Professional Development
•Participate in ongoing training related to family support, trauma-informed practice, domestic-violence informed practice, diversity, and early childhood development
•Engage in reflective practice and continuous learning
•Maintain awareness of best practices and emerging trends in family support services
• Actively work toward achieving goals outlined in their Professional Growth Plan

Working Conditions
•Work is conducted in family homes, community environments, and office settings
•Exposure to complex family situations requiring emotional resilience and professionalism
•Ability to maintain appropriate professional boundaries in home-based environments
•Flexible hours required to meet the needs of families (including evening & weekends as required)
•Ability to meet the physical and mobility demands of community-based work, including travel between sites and delivery of programming in a variety of environments and conditions
•Ability to safely drive in winter and variable road conditions, including monitoring travel advisories to plan for road and weather conditions between program sites
•This is not a remote position; regular travel and in-person presence at community sites is required

Salary: $50,000-55,000.

Position will remain open until a successful candidate has been found.
