The Case Manager provides comprehensive home-based services to help residents stabilize in permanent housing and increase their income and resources. Responsibilities include conducting assessments, developing individual service plans, and documenting all client contacts in HMIS.
Candidates must possess a reliable vehicle and car insurance for travel throughout the City of Chicago. Strong organizational skills, professional boundaries, and a passion for serving underhoused populations are required.

ORGANIZATION SUMMARY
Facing Forward to End Homelessness (Facing Forward) ends the cycle of homelessness for families and
individuals by offering hope, help, and housing. Facing Forward utilizes a Housing First model, which
provides permanent supportive housing to chronically homeless individuals, families, and veterans.
Under this model, we first ensure that clients have a safe, stable home, and then we empower them as
they lead a forward-facing life. By providing permanent housing and a full array of services, including
case management, healthcare, referrals for substance abuse treatment, mental health issues, education,
and job training, we give homeless people in Chicago the tools they need to reclaim their lives.

We partner with families and individuals who have experienced chronic homelessness and are living with a disability and empower them with the stability of a stable home and the tools they need to increase their self-sufficiency and become confidently independent. By implementing the “Housing First” approach, we are currently ending homelessness for over 660 people in Chicago, transforming lives by addressing first and foremost the need for a safe and stable home. An individual’s basic needs for food and shelter must be met before successfully addressing root causes of chronic homelessness. We provide long-term housing at the outset of service, allowing for rapid outreach to the most vulnerable populations. Once this first step toward stability has been taken, we provide client-directed case management services to support the journey toward self-sufficiency. Facing Forward provides a full range of client-directed case management services focused on mental and physical health, life skills, education, and employment.
