Your opportunity
As a Care Partner, you’ll be a trusted advisor and advocate, supporting clients to navigate services and funding so they can maintain their independence and quality of life in their own homes.
You’ll make an impact by
- Conducting holistic assessments and developing personalised care plans aligned to each client’s goals
- Building strong, meaningful relationships with clients, families, and stakeholders
- Coordinating services and responding proactively to changing needs
- Supporting clients to understand and effectively utilise their budgets and funding
- Managing competing priorities in a fast-paced environment while maintaining high-quality outcomes
This role will suit someone who is resilient, adaptable, and confident navigating challenges, with a genuine passion for helping others.
What you’ll bring
To succeed in this role, you’ll need the following qualifications:
- Tertiary qualifications in the Human or Behavioural Sciences, Allied Health, Aged Care or Community Services Fields
- Experience in client case management
- National Police check
- Driver’s licence

- Strong communication skills with clients, families, and colleagues
- Organisation and the ability to manage competing priorities
