Act as a trusted advisor and advocate for clients to help them navigate services and funding for independent living. Responsibilities include conducting holistic assessments, developing personalized care plans, and coordinating services to meet changing client needs.

Requires tertiary qualifications in Human/Behavioural Sciences, Allied Health, Aged Care, or Community Services, along with experience in client case management. Candidates must possess a National Police check and a valid Driver's licence.

This is a great opportunity for recent graduates to launch their career in a customer-focused role as a trusted advisor to clients, making a genuine difference in people’s lives.

Your opportunity

As a Care Partner, you’ll be a trusted advisor and advocate, supporting clients to navigate services and funding so they can maintain their independence and quality of life in their own homes.

You’ll make an impact by

  • Conducting holistic assessments and developing personalised care plans aligned to each client’s goals
  • Building strong, meaningful relationships with clients, families, and stakeholders
  • Coordinating services and responding proactively to changing needs
  • Supporting clients to understand and effectively utilise their budgets and funding
  • Managing competing priorities in a fast-paced environment while maintaining high-quality outcomes

This role will suit someone who is resilient, adaptable, and confident navigating challenges, with a genuine passion for helping others.

What you’ll bring

To succeed in this role, you’ll need the following qualifications:

  • Tertiary qualifications in the Human or Behavioural Sciences, Allied Health, Aged Care or Community Services Fields 
  • Experience in client case management 
  • National Police check  
  • Driver’s licence
