Your opportunity

As a Care Partner, you’ll be a trusted advisor and advocate, supporting clients to navigate services and funding so they can maintain their independence and quality of life in their own homes.

You’ll make an impact by

• Conducting holistic assessments and developing personalised care plans aligned to each client’s goals
• Building strong, meaningful relationships with clients, families, and stakeholders
• Coordinating services and responding proactively to changing needs
• Supporting clients to understand and effectively utilise their budgets and funding
• Managing competing priorities in a fast-paced environment while maintaining high-quality outcomes

This role will suit someone who is resilient, adaptable, and confident navigating challenges, with a genuine passion for helping others.

What you’ll bring

To succeed in this role, you’ll need the following qualifications:

• Tertiary qualifications in the Human or Behavioural Sciences, Allied Health, Aged Care or Community Services Fields 
• Experience in client case management 
• National Police check  
• Driver’s licence

• A natural ability to connect and build relationships with diverse individuals
• Genuine empathy and respect for different backgrounds and life experiences
• Strong communication skills with clients, families, and colleagues
• Organisation and the ability to manage competing priorities

• Confidence (or willingness to learn) in supporting client budgets and funding

• A collaborative approach and strong team focus
