POSITION DESCRIPTION
JOB TITLE: Prevention & Referral Specialist
REPORTS TO: Prevention Services Manager
POSITION OVERVIEW:
The Prevention & Referral Specialist is responsible for providing individuals with health information, referrals, testing and treatment, risk reduction interventions, condom distribution, and awareness. Position also follows up on referrals to assist individuals with overcoming barriers and ensure linkage to services.
RESPONSIBILITIES AND DUTIES:
Provides group-level facilitation.
Performs Linkage to Care activities.
Provides HIV/HCV counseling and testing Services at Center and in field locations.
Conducts Quality Assurance, including, but not limited to, daily opening of testing labs and documentation of temperatures
Provides service navigation.
Tracks referrals and linkages
Provides partner services/disclosure assistance.
Provides risk and harm reduction counseling
Maintains organization of risk/harm reduction materials.
Performs office/Center duties as needed.
Collects client-level and aggregate data.
